An express report on the "6•17" personal injury accident in a 110 kV substation

On June 17, 2021, a power supply company encountered an electric shock accident during the construction of a 110 kV substation power distribution device transformation project, resulting in the death of one construction unit and one injury to the operation and maintenance personnel of the substation.
 
1. Project Overview
 
The incident was the transformation of 110 kV power distribution equipment in a 110 kV substation of a certain company. The content of the transformation was to replace the 110 kV indoor open handcart circuit breaker. The winning bidder was the power installation branch of a group company (provincial management industry). Unit, referred to as "installation branch"). In order not to affect the power supply of important users, the substation will be operated in the transition mode of Ⅰ Hongqi Provincial Government Line directly connected to the provincial 1# substation and II Feng Provincial Line directly connected to the provincial 2# substation during the transformation period. After the transformation was completed on June 12, the 110 kV bus resumed operation. (The operation mode is shown in Figure 1 and Figure 2)

2. The accident
 
According to the work plan, from 21:00 on June 16th to 18:00 on June 17th, the construction unit carried out the transition from the 110kV II Feng Provincial Line to the Provincial 2# main transformer in a certain substation, and the work site was in Feng II. The outer side of the provincial line through the wall bushing (outdoor line side). At 17:50 on June 17, the work was over and the work was completed.
At this time, the 110kV II Fengsheng 2 switch is in cold standby state, and the bus side of the II Fengsheng 2 Dongdao gate is live. At 19:13 on June 17th, Sun Mou, the person in charge of the installation branch, and the substation operation and maintenance were when Shen Mou went to II Feng Province 2 switch interval, illegally using steel tape measure for measurement work (not listed in the work ticket) , Discharge occurred when the steel tape was close to the live busbar, causing Sun to be electrocuted and Shen to be severely burned. 3. The problems of accident exposure and related requirements
 
After preliminary investigation and analysis, the accident revealed that the province's power company did not implement the national and company safety production deployment in place, and there were many weak links and problems in safety production management. One is the inadequate ideological understanding. The accident occurred at the critical stage of the company’s special rectification of production safety and the “Five Inspections and Five Strict” special activities. It occurred during the celebration of the 100th anniversary of the founding of the Party and the National Work Safety Month, reflecting the extreme importance of the accident unit to ensuring safe production. The understanding is not in place, the real attention is not paid to the ideology, the implementation of responsibilities is not strict and meticulous, and the on-site safety is out of control and control. Second, the lessons of the accident were not learned profoundly. Since May, there have been two consecutive safety production accidents in the company’s production field. The company has issued newsletters and notifications, and organized "clear" meetings, requiring all units to study and implement them. However, the company has not effectively learned the lessons of the accident. Safety learning is just a mere formality. , Resulting in repeated occurrences of similar accidents. Third, the identification of security risks is not in place. The description of "work location" in the work ticket is inaccurate, the analysis of risk points is incomplete, the live parts of the equipment are incomplete, the isolation measures are not specifically arranged, and the warning signs are hung; the on-site safety briefing is mere formality, and the relevant management personnel are not strictly implemented. Require. Fourth, the safety awareness of operators is weak. The person in charge of on-site work and operation and maintenance personnel violated the "Safety Regulations", used steel tapes to conduct measurement in the live equipment area illegally, and entered the equipment area without a safety helmet, and lacked basic safety awareness. Fifth, the awareness of legal compliance is not strong. Relevant units and personnel of the provincial company violated the company's "Safety Accident Investigation Regulations" safety information submission work requirements, and failed to report the accident information to the company headquarters within the specified time.
 
At the moment when the company celebrates the 100th anniversary of the founding of the Communist Party of China, it is a critical period for ensuring power, and the responsibility for ensuring safety is heavy. Provincial power companies must deeply reflect, immediately rectify, and comprehensively investigate the weak links in safety management. At the same time, in accordance with the principle of "Four Don't Let Go", we must seriously organize accident investigation and analysis, deal with the accident-related units and responsible persons seriously, and do a good job in the investigation and rectification of deep-level problems in safety management. All units of the company should take the accident as a mirror, draw inferences from one another, and take resolute and effective measures to prevent similar accidents from happening again. One is to earnestly learn from the accident. All units should study "5.2", "5.4", "6.17" and other accident bulletins (notices) in combination with thematic safety day activities, check and investigate the weak links of safety management in their professional field, and implement rectification and preventive measures. The second is to strengthen accountability and accountability. Members of the leadership team at all levels should conscientiously perform their responsibilities according to the "two lists", and do a good job in production safety in accordance with the requirements of "one post with two responsibilities" and "three managements and three musts"; management personnel at all levels should follow the list of safety responsibilities for their posts and perform strictly Management responsibility and supervision responsibility; in case of an accident, it shall be held accountable. The third is to strengthen the safety management of overhaul operations. Maintenance units and operation and maintenance units must earnestly perform their safety responsibilities, strictly compile and implement the "three measures and one case" of overhaul construction, and earnestly grasp the organization and management of operations. Standardize the implementation of the "two votes and three systems", strictly implement the "five prevention" measures, strengthen the analysis and control of dangerous points, and consolidate the safety management foundation for maintenance operations. The fourth is to strengthen the safety supervision of the provincial management industry. Strictly perform the safety management responsibilities of the sponsors and professional departments and the main safety responsibilities of the provincial-administered industrial units, set up safety management departments as required, arrange all kinds of safety management personnel, implement homogeneous management requirements, and improve the safety production organization system and institutional system. Strengthen the safety education and training of construction workers, strictly enforce the safety access of industrial units and personnel, and increase the safety management and control of projects undertaken by industrial units. Fifth, the implementation of the "four controls" is strictly enforced. Give full play to the role of "one platform, one center, one team, one terminal", continue to carry out "four nos and two direct" safety inspections, strictly carry out supervision and inspection, evaluation and assessment, and earnestly implement the "four management" risk prevention measures . Sixth, strict reporting of accident information. After an accident occurs, relevant units must strictly implement the company’s "Safety Accident Investigation Regulations" that the reporting time for each level should not exceed one hour, and resolutely eliminate late reporting and concealment. Units and individual companies that have delayed or concealed reports will be upgraded in accordance with the "Safety Work Reward and Punishment Regulations", and the main planners and decision makers will be held accountable.
